Note: Payments are debited/charged the first of the month. If you wish to cancel care, written notice is required (forms are at each site). Cancellations must be received by Kids Choice no later than 15 days prior to the next scheduled billing date. If you cancel care sometime during the current month’s care, that month’s tuition will not be refunded, nor credit placed onto the account; the account would be cancelled the first of the next month. AUTHORIZATION FOR EMERGENCY MEDICAL CARE

I, the undersigned, understand that if emergency medical care is deemed necessary by a medical physician and that if I cannot be contacted, I hereby authorize my Kids Choice child care provider to act on my behalf in granting permission for my child(ren) to receive treatment as specified: In the event of a serious injury the local Ambulance Squad will be contacted immediately. After this initial call, contact will be made to the parents or the emergency contact. The director or the onsite supervisor will accompany the child to the emergency room and await the arrival of the parents or emergency contact. If a child is experiencing an illness he or she will be attended to by a Kids Choice employee and the parent or emergency contact will be contacted.

Policy Injury Reporting Procedures

It is the policy of Kids Choice that parents/guardians shall be notified of all injuries that require care. The method and means of contact shall be done in the following manner;

(1) The parent shall be called and informed via phone of the incident and what actions are being taken to address the injury, ex. Ice being applied, observation, etc. (2) If you would like a copy of the written report we will provide on for you within 24 hours, unless that falls over the weekend or holiday. In that case, you will receive documentation on the next business day.

As per new State guidelines, Kids Choice will be adopting some new procedures regarding electronic devices and usage. Electronic devices such as Nintendo DS and small iPads are permitted but must be limited to educational and instructional age-appropriate use. Also, as per the State, the devices cannot be utilized in place of planned activities or passive doings.

It is the policy of Kids Choice that electronic equipment and devices can be utilized in the program under the following conditions:

If planned activities are organized, no electronic devices shall be utilized
Limited usage not to exceed 20 minutes per session

The use of cell phones or smart watches will not be allowed during Kids Choice or KC Camp. This is for the safety of your child, as well as the other children and Kids Choice staff. If a child is counseled on using either of these items during operating hours, we will take the device and return it to the parent at pickup. If your child needs to speak with you, they can ask the staff at any time to contact you.

Kids Choice is not responsible for any lost, stolen, or damaged items at Kids Choice.

Pick Up / Release of Children Policy on Pickups

Kids Choice operates from school dismissal till 6 p.m. Parents/guardians are permitted to pick up their child at any time during those hours. Late Pickups; for children that have not been picked up by 6:00 pm a $25.00 late fee will be assessed. If not picked up by 6:30 pm an additional $15.00 fee will be charged, and the emergency contacts will be contacted. The Child Abuse Hotline 1-877-NJ-ABUSE (1-877-652-2873) will be contacted at 7:00 pm and the local Police Department.

Kids Choice has implemented safety procedures to ensure that unauthorized persons are not permitted in the building and/or do not have a child released to him/her. To gain access to the building, the individual must knock on the locked door. Kids Choice uses the peephole prior to opening the door. If the person who knocks is not recognized by the Kids Choice staff that person is not permitted entry. Instead, the Kids Choice staff member will leave the building (with another Kids Choice Staff member watching through the peephole) to ascertain who is seeking entry and for what purpose. Should the person be an unauthorized person, they will neither be permitted entry nor permitted to remove any child. If a particular child is sought, that parent/guardian of that child shall be immediately notified. The Washington Township Police Department shall be notified by calling 856-256-1212.

In the event of a restraining order or a custody order, the parent/guardian will provide Kids Choice a copy of the Order. All Kids Choice staff will become familiar with the Order. Should the person be an unauthorized person, they will neither be permitted entry nor permitted to remove any child. The main office will be notified as well as The Washington Township Police Department by calling 856-256-1212. Should Kids Choice believe that the person seeking to pick up the child may place the child at risk; Kids Choice shall notify the Washington Township Police Department and request their assistance.

If a child is not picked up by closing time, Kids Choice will contact authorized and emergency contacts. An hour or more after closing - and child still has not been picked up; Kids Choice staff member will contact 24 Hour Child Abuse Hotline.
